Physical Therapist (PT) – Home Health | Waianae, HI | Part-Time & PRN | $65–$85 Per Visit

Are you a licensed Physical Therapist looking for flexible work with strong per-visit pay and control over your schedule? Stern at Home Therapy is hiring Part-Time and PRN Physical Therapists in Waianae, HI to provide one-on-one care to patients in their homes.

This is a great opportunity for a PT who wants to build a consistent local caseload or add flexible visits around an existing schedule without the double-booking, productivity quotas, or clinic politics common in traditional outpatient settings.

What You'll Do:
As a Physical Therapist with Stern at Home Therapy, you will evaluate and treat a primarily geriatric patient population in their homes throughout the Waianae area. You’ll have the clinical autonomy to develop individualized plans of care while our administrative team supports you with scheduling, referrals, authorizations, and patient coordination.

Responsibilities:
  • Perform physical therapy evaluations and develop individualized plans of care.
  • Provide one-on-one physical therapy treatments in patients' homes.
  • Help patients improve strength, balance, mobility, endurance, and functional independence.
  • Provide gait training, transfer training, fall-prevention strategies, and home exercise programs.
  • Recommend assistive devices, durable medical equipment, and home safety modifications when appropriate.
  • Complete accurate and timely clinical documentation.
  • Communicate with physicians, caregivers, families, and members of the interdisciplinary care team.
  • Manage your caseload around your availability.

Compensation:
Part-Time W-2 — Minimum 10 Patients
  • $65 per follow-up visit
  • $75 per 4-unit evaluation
Part-Time 1099 — Minimum 10 Patients
  • $72 per follow-up visit
  • $85 per evaluation
PRN — Minimum 5 Patients
  • $72 per follow-up visit
  • $85 per evaluation

Part-time and PRN compensation is paid at a fixed per-visit rate and does not use a tiered volume model.
