  • POSITION INFORMATION
    • Position summary:
      • The Hospice Social Worker is responsible for providing initial psychosocial assessments, plan of care, ongoing psychosocial counseling, bereavement services, community education and outreach and referral to Hospice patients and families.
  • MINIMUM QUALIFICATIONS
    • Education:
      • Master's Degree in Social Work and is a Licensed Independent Social Worker.
    • Required length and type of experience:
      • Minimum of three (3) years of clinical work experience preferably in a healthcare setting or other related field.
    • Required licensure, certification or registry:
      • Licensed Independent Social Worker.
