Community Outreach Assistant (Virtual Assistant)

PRN Part-Time Independent Contractor | Remote | Local Maryland Hire Only


About the Role

We are a mission-driven non-medical home care agency based in Maryland dedicated to helping individuals remain safe, independent, and comfortable in their homes.


Our work supports smooth transitions from facility to home and helps connect clients with high-quality, reliable care in the community. We are seeking a professional and personable Community Outreach Assistant to help our home care agency build relationships with healthcare facilities and community partners.


This role focuses on outbound outreach and partnership development with skilled nursing facilities, rehab centers, hospitals, assisted living communities, and healthcare professionals.

The ideal candidate is confident on the phone, organized, professional, and comfortable communicating with healthcare staff and referral sources.


Responsibilities (but not limited to the following):

  • Serve as a liaison between the agency and healthcare community partners to support smooth transitions from facility to home
  • Conduct inbound and outbound calls and send follow-up emails to healthcare facilities and community organizations
  • Introduce the agency as a potential home care resource for patients transitioning to home
  • Build and maintain professional relationships with discharge planners, social workers, case managers, and referral coordinators
  • Communicate via phone and email to inquire about referral processes and preferred provider or community resource lists
  • Identify partnership development and referral opportunities through phone and email outreach
  • Maintain organized outreach documentation and complete weekly activity tracking
  • Submit weekly outreach summaries and updates via email
  • Attend mandatory weekly remote team meetings (in-person meetings and training may also be required)

Weekly Expectations

  • Maintain consistent outbound outreach, follow-ups, and email communication with healthcare partners
  • Professionally represent the agency in all phone and email communication
  • Ensure accurate documentation, email tracking, and follow-up logs for all contacts and activities
  • Support long-term partnership development and referral relationship growth

Qualifications

  • Must be 18 years of age or older
  • Local Maryland hires only; candidates must reside in Maryland and be available for in-person meetings and training as required
  • 1–2 years of experience in healthcare, home care, healthcare marketing, community outreach, customer service, or a related field preferred
  • Strong communication and relationship-building skills
  • Professional phone etiquette
  • Organized and detail-oriented
  • Comfortable speaking with healthcare professionals and administrative staff
  • Reliable internet and computer access
  • Proficiency in Microsoft Excel, Word, email, Office 365, and other technology platforms for communication and documentation
  • Ability to maintain confidentiality

Position Details

  • Remote independent contractor position
  • Up to 6 hours per week (hours are not guaranteed)
  • Work hours must be completed between 10:00 AM and 3:00 PM on scheduled workdays - Flex Schedule
  • Occasional in-person office meetings and training required (Maryland-based)
  • Consistent weekday daytime availability required
  • Calls made through company phone system

Additional Terms

  • Position includes an introductory contract period of up to 10 days
  • Must successfully pass background clearance, medical screening and drug screening may be required prior to starting
  • Contractor must sign confidentiality agreement
