Key Responsibilities
  • Conduct thorough evaluations of patients in their home environments, assessing their physical limitations, mobility, and overall functional capacity, using standardized assessments as well as observational techniques.
  • Develop individualized treatment plans that reflect each patient’s unique needs and therapeutic goals, employing a mix of therapeutic exercises, hands-on techniques, and the use of specialized equipment.
  • Utilize diverse methodologies such as strength training, manual therapy, and pain management techniques to facilitate recovery and improve patient outcomes.
  • Document and track patients’ progress meticulously, updating treatment plans as necessary to reflect any changes in condition or response to therapy.
  • Provide education and training to patients and their families on physical therapy techniques, assistive devices, and self-management strategies to promote independence.
  • Regularly reassess patients, adapting treatment strategies accordingly to ensure optimal recovery trajectories.
  • Stay current with industry best practices and advancements through ongoing professional development, including workshops and relevant literature.
Patient Population / Client Focus
