Job description

Primary Responsibilities:

  • Directly supports the High-Risk Prioritization (HRP) and Transitions of Care (TOC) programs, which are critical to improving patient outcomes, reducing avoidable utilization, and ensuring regulatory compliance
  • Conducts outreach and care coordination activities for a panel of more than 30,000 empaneled patients, supporting the operational, clinical, and quality performance of all Colorado-employed Internal Medicine and Family Practice (IMFP) clinics
  • Manages a substantial and complex monthly workload, including completion of 100-250 post-hospital discharge follow-up calls and development of individualized patient profile sheets outlining clinical concerns and care considerations for approximately 140 Primary Care Providers (PCPs)
  • Supports provider productivity, patient satisfaction, quality gap closure initiatives, revenue generation, and achievement of organizational performance measures, including HEDIS and Medicare Advantage quality goals
  • Assists in identifying and addressing care gaps to improve patient outcomes, enhance the patient experience, support National Patient Satisfaction (NPS) objectives, and contribute to achievement of 5-Star quality ratings
  • Supports organizational growth and Medicare Advantage initiatives through timely patient engagement, care coordination, and effective management of transitions of care
  • Serves as the patient's first point of contact within the clinic, demonstrating C-I-CARE principles while providing exceptional customer service
  • Performs patient check-in and check-out functions, verifies insurance eligibility and benefits, answers and routes incoming calls, schedules appointments, and collects patient copays and outstanding balances in accordance with organizational policies and procedures
  • Ability to work independently
  • Current schedule- Monday-Friday 8am-5pm Mountain Time

Required Qualifications:

  • Completion of a Medical Assistant program
  • Basic Life Support (BLS) certification from AHA or willing to obtain by date of hire
  • 2 years of experience working as a Medical Assistant


Preferred Qualifications:

  • 3 years of experience in a multi-provider setting
  • Experience working remotely 
  • Electronic Medical Records (EMR) experience
  • Bilingual English and Spanish

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $18 - $32 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
