Job description

Pediatric On-Call RN

The Pediatric On-Call Registered Nurse (RN) provides compassionate, developmentally appropriate, family centered care to infants, children, and adolescents with a variety of medical conditions. The Pediatric After Hours RN works as a part of a multidisciplinary team to coordinate physical, emotional, and spiritual care of the patient and family.

Key Responsibilities:

  • Provide skilled nursing care and on-call support for pediatric hospice patients during nights, weekends, and holidays
  • Respond to patient and family needs, including medical and emotional crises, with timely home visits
  • Communicate patient status changes and collaborate with physicians and the interdisciplinary team
  • Complete hospice admissions and coordinate care in alignment with the plan of care
  • Educate families and caregivers on symptom management, medications, and care techniques
  • Maintain accurate, timely clinical documentation and point-of-care records
  • Coordinate services, supplies, and referrals, including medications and durable medical equipment (DME)
  • Travel-based role providing in-home patient visits
  • Schedule from 8:00p.m. to 8:00 a.m.: (3) 12-hour shifts with rotating weekends and holiday coverage

Education/Experience:

  • Current Illinois RN license
  • 2+ years nursing experience (1+ year pediatric required)
  • Pediatric med-surg skills required
  • Home care or hospice experience preferred
  • Valid driver's license and insured, reliable vehicle
