Job Summary

We are seeking a dedicated and compassionate Registered Nurse (RN) to join our interdisciplinary team. The RN will provide exceptional nursing care to residents, ensuring their physical, emotional, and social needs are met in accordance with individualized care plans.

Key Responsibilities
  1. Deliver patient-centered nursing care to residents in a long-term care and rehabilitation setting.
  2. Assess, monitor, and document residents' health conditions and progress.
  3. Administer medications and treatments as prescribed, ensuring compliance with facility protocols.
  4. Collaborate with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans.
  5. Provide education and support to residents and their families regarding care plans and health conditions.
  6. Maintain accurate and timely documentation in compliance with state and federal regulations.
  7. Foster a safe, comfortable, and therapeutic environment for residents.
  8. Supervise and mentor nursing assistants to ensure quality care delivery.
Qualifications
  1. Valid Registered Nurse (RN) license
  2. Strong clinical skills and knowledge of geriatric and rehabilitation care.
  3. Excellent communication and interpersonal skills.
  4. Ability to work collaboratively in a team-oriented environment.
  5. Compassionate, patient-focused approach to care.
  6. Current BLS/CPR certification required; ACLS certification a plus.
