Pride health is actively hiring a Registered Nurse to join our client's home health program in a growing care team serving patients.

Compensation:

  • Local Pay: $55.00 to $57.00/hour (W-2)

Mileage Reimbursement: $0.38/mile (First Patient to the last patient)

Location: Syracuse, NY, 13212 (Coverage area: Surrounding locations agency service area.)

  • Shift: 8 Hours (Day Shift)
  • Guaranteed Hours: 40 Hours/Week
  • Contract Duration:  5+ Months (Potential for an extension)
  • Schedule: Monday to Friday, 09:00 AM to 05:00 PM, 8 hours per day

Job Summary:

Registered Nurse (RN) – Home Health to provide high-quality skilled nursing care to patients in their homes. The RN will conduct comprehensive patient assessments, develop individualized care plans, coordinate care with the interdisciplinary team, and ensure accurate OASIS documentation while delivering compassionate, patient-centered care.

Key Responsibilities (KRAs):

  • Conduct comprehensive home health assessments, complete OASIS documentation, develop and implement individualized care plans, administer skilled nursing care, and coordinate patient care with physicians and the interdisciplinary team to ensure optimal clinical outcomes.
  • Maintain accurate Homecare Homebase (HCHB) documentation, educate patients and caregivers, monitor patient progress, ensure regulatory compliance, supervise home health aides as needed, and provide on-call support while delivering high-quality, patient-focused care.
  • Required Certifications/Licenses/Skills
  • Active Registered Nurse (RN) license in the state of practice with current CPR/BLS certification, valid driver's license, reliable transportation, and current auto insurance.

Required Qualifications:

  • Graduate of an accredited associate degree in nursing (ADN) or Nursing Diploma program (BSN preferred).
  • Minimum 1–2 years of clinical nursing experience as an RN.
  • OASIS documentation experience is required.
  • Ability to work independently while collaborating effectively with an interdisciplinary team.
  • Willingness to travel locally for patient visits and participate in an on-call rotation.
  • CHPN (Certified Hospice and Palliative Nurse) certification is preferred.

Essential Duties:

  • Perform comprehensive nursing assessments in patients' homes.
  • Develop, implement, and evaluate individualized Plans of Care (POC).
  • Complete timely and accurate OASIS assessments and clinical documentation.
  • Administer medications, IV therapy, wound care, catheter care, and other skilled nursing interventions.
  • Monitor patient condition and communicate changes to physicians and care teams.
  • Educate patients and caregivers on disease management, medications, home safety, and self-care.
  • Coordinate referrals and collaborate with interdisciplinary team members.
  • Supervise Home Health Aides (HHAs/LVNs/STNAs) when applicable.
  • Ensure compliance with infection control standards, agency policies, and payer requirements.
  • Participate in on-call responsibilities and interdisciplinary meetings as assigned.
