SUBJECT:ย  Application Procedures for Substitute Teaching

We look forward to receiving your application to substitute teach in the Oldham County Schools! There are several requirements that must be completed before you are eligible to substitute in our district. Please complete each of the steps below:

  1. ย Professional Certified Staff Applicationย - including 2 References (online).
  2. If you hold a valid Kentucky Teaching Certificateย - Upload with your application or provide to Personnel Officeย (if applicable).
  3. Official College Transcript(s) and/or High School Diplomaย  Upload with your application or provide to Personnel Officeย .
  4. Medical Examination and TB Risk Factor Assessment This is your financial responsibility.ย  You may see any doctor of your choosing.ย  Print form, visit your doctor and provide to Personnel Officeย .
  5. Authorization to Release Information Will send electronically.
  6. Employment Eligibility Verificationย (Form I-9)ย  Complete Section 1 only of this form; attach a copy of your driver's license and social security card for identification purposes andย provide to Personnel Officeย . Will send form electronically.
  7. Child Abuse or Neglect Checkย - CANS checks (Child Registry Check DPP-156) must be requested by the applicant through the Kentucky Online Gateway:ย  https://kog.chfs.ky.gov/home/ย .ย  When requesting a CANS check, please check the box for Employer/Agency Information and list for Employer:ย  Oldham County Schools and Email Address:ย  katie.garrett@oldham.kyschools.us.ย  Follow the instructions to complete the Central Registry Check (Form DPP-156) online.ย  A $10 payment is required and you will have to upload a copy of one of the following: driver's license, birth certificate or social security card.ย If you have any questions about completing the form, please call the Help Desk in Frankfort at 502-564-0104 ext. 2.
  8. Form W-4ย andย K-4ย - Will send electronically.
  9. School Preference Formย - Will send electronically.
  10. Automatic Deposit Formย - Will send electronically. Will also need a voided check.
  11. Electronic Materials Code of Ethics Agreementย - Complete and provide to Personnel Office.
  12. Form SSA-1945ย - Will send electronically.
  13. FBI Fingerprint Checkย  This is your financial responsibility. The cost is $54.00.
