Job description

American Traveler is seeking a Home Health Physical Therapist Case Manager with at least 1-2 years of recent home health experience with adult/geriatric patients for a 13-week travel contract in the Chicago, IL metro area.
Job Details
• 100% remote field-based position within a designated coverage area in the Chicago metro area (Central Metro E), following an approximately 2-day onsite orientation in Oak Brook, IL,
• All team meetings are held virtually; there is no physical office to report to,
• Coverage zip codes include: 60630, 60618, 60625, 60646, 60659, 60660, 60657, 60640, 60613, 60614, and surrounding areas,
• Current team census is approximately 200 patients; patient population is adult and geriatric general med/surg,
• Expectation of 6 patient visits per day within the coverage area,
• Schedule is Monday-Friday, 8:00 AM-5:00 PM; no weekend rotation, no on-call, and no holiday requirements,
• EMR: Epic (experience required); OASIS documentation experience required,
• Supplies and equipment will be delivered to the traveler's home,
Job Requirements
• Minimum 1 year of recent home health experience required; 2 years preferred, specifically with adult/geriatric patients,
• Active IL PT license required (license required if applicable),
• Current BLS certification (AHA) required,
• Valid driver's license and active auto insurance required; reliable personal transportation is mandatory for patient visits,
• Experience with Epic EMR required,
• OASIS documentation experience required,
• Proficiency in assisting patients with ADLs and adaptive equipment,
• Ability to effectively collaborate with interdisciplinary team members including RN, OT, and Speech,
Additional Information
• Responsibilities include independent patient evaluation and treatment visits across the designated home health coverage area,
• Orientation covers EMR training and skills check-off,
• Team support includes a nursing supervisor, manager, and team leads,
• Any solid color scrubs are acceptable; business casual attire is also permitted - no jeans or open-toe shoes,
• Candidate must provide their own transportation to and from all patient visits,
• First-time travelers will be considered for this position if they are the right candidate,
• Local candidates are accepted at the same rate; no radius rules apply,
• Former employees of Advocate Health or any of its subsidiaries must be separated for at least 12 months before being eligible,
• At least 1 professional reference dated within the past year is required; must be from a Supervisor, Manager, Director, or Charge RN
