Job description

MININUM QUALIFICATIONS

  • Minimum 16 hours of yearly training
  • First Aid certified and CPR certified
  • Clean driving record/Clean Criminal Record Check
  • Must be at least 18 years old
  • Have a high school diploma or General Education Diploma
  • Must be able to pass a Physical and free of all communicable diseases
  • Must meet COMP Section 905 transportation requirements
  • Sensitivity to the service population’s cultural and socioeconomic characteristics is required

DUTIES AND RESPONSIBILITIES (HOUSE MANAGER)

  • Manager 1-3 Residential Group Home Sites.
  • Manages direct care staff in group home(s) and ensure that delivery of services is congruent with Individual Service Plans and requirements of provider, licensing, and funding agencies.
  • Provide direct care to individuals in services. Provide training or assistance in household care, such as meal preparation, clothes laundering, bed-making, housecleaning, shopping, and other similar tasks
  • Be On-Call for emergency situations and issues from home staff two weeks each month. Evaluate issues and respond to appropriately.
  • Actively participate in ISP, SIS and other like participant centered meetings.
  • Completes residential and community activity schedules and ensures their implementation.
  • Ensure clinical documentation is accurate, complete and timely to compliance with agency and regulating bodies with daily audit.
  • Provide program summaries as needed.
  • Conduct regular home inspections and participant record audits.
  • Ensure that each residents nutritional needs are being met by preparing a weekly menu distribute food and supply items weekly and as needed to home.
  • Ensure identification of possible individual’s change in condition with swift and appropriate action.
  • Ensuring all participants medical issues are addressed appropriately and appointments are attended, documented correctly and timely.
  • Transport and attend appointments as necessary and communicate effectively with and consult with medical professionals concerning participant treatment and care.
  • Manage staff in abiding by company policies and procedures as well as provider, state, federal, and funding agency guidelines.
  • Ensure Safety and Universal Precautions practices are followed by participants and staff as necessary to follow health and safety practices.
  • Ensure that each program’s housekeeping, dietary, personal care and other needs are being met..
  • Provide orientation training for new staff and on-going training and support of staff.
  • Ensure staff is working as a team.
  • Ensure appropriate staff coverage at all times within budget and staffing guidelines, Manage OT requests. Reassign staff as required, with supervisor prior authorization. Substitutes for Direct Care Staff as needed.
  • Provide regular participants and staff meetings.
  • Ensure proper handling of petty cash, participant allowances, receipts and related documentation.
  • Participate in provider meetings and relevant conferences.
  • Perform related duties as needed and assigned.
  • Assisting with therapeutic exercises, supervising self-administration of medication and performing other medically related services;
  • Implements the behavioral support plans of participants to reduce inappropriate and/or maladaptive behaviors and to acquire alternative adaptive skills and behaviors.
  • Assists with the Quality Improvement Program.
  • Plans for and adheres to the general health/safety practices necessary for the welfare of the person served.
  • Attends and participates in in-service trainings.
  • Transfers service recipients to and from wheelchair, bathroom facilities, vehicle and performs any other transfers, as required.
  • Transports program participants as needed using personal auto(w/ millage reimbursement) or agency vehicle.
  • Performs any other duties deemed necessary by the supervisor and Director of LPC.

Typically Monday- Friday 11am-7pm. Flexible 8 hour shifts. On-call 1 week a month.
