Remote | Evenings | Overnights | Weekends | Holidays
Per Diem / PRN Position / 1099 Independent Contractor Role
Job Objective
VeriCore Consulting Group is seeking experienced Registered Nurses to provide overnight clinical triage support for Assisted Living and Memory Care communities across multiple states.
The Triage RN is responsible for delivering sound clinical judgment, exceptional communication, and efficient problem-solving through remote triage support for onsite staff. This role requires the ability to assess incidents, changes in resident condition, and staff concerns through high-volume telephonic communication while maintaining professionalism, accuracy, and calm decision-making under pressure.
Additionally, the Triage RN will coordinate outbound communication with hospice agencies, home health providers, pharmacies, physicians, families, and other healthcare partners as needed.
Qualifications
  1. Current RN license in good standing
  2. Compact - Multi-state licensure required
  3. Minimum of 5-7 years of combined experience in:
    1. Assisted Living
    2. Memory Care
    3. Long-Term Care
    4. Skilled Nursing
    5. Geriatric or Senior Care
  4. Strong clinical assessment and investigative skills
  5. Excellent verbal and written communication abilities
  6. Strong organizational skills with the ability to work independently and remain self-accountable in a remote environment
  7. Experience with Electronic Health Record systems preferred, including:
    1. ECP
    2. PointClickCare
    3. Eldermark
    4. Epic

    6. Residex
  8. Experience with telehealth and remote communication technologies preferred
  9. Strong working knowledge of:
    1. Pharmacology
    2. Blood glucose monitoring

  10. Ability to remain calm, professional, and solution-focused during emergent situations
  11. Team-oriented mindset with flexibility to assist in schedule coverage as operationally needed

Essential Responsibilities
  1. Answer incoming triage calls during scheduled shifts, including evenings, overnights, weekends, and holidays
  2. Assess resident incidents, falls, medication concerns, behavioral changes, and emergent clinical situations remotely
  3. Provide guidance and direction to onsite caregiving teams regarding appropriate interventions and escalation procedures
  4. Make outbound calls to:
    1. Hospice agencies
    2. Home health providers
    3. Pharmacies
    4. Physicians
    5. Families
    6. Emergency contacts
  5. Document all interactions and recommendations appropriately within the Electronic Health Record
  6. Maintain HIPAA compliance and confidentiality standards at all times
  7. Communicate effectively and professionally with a diverse workforce and healthcare partners
  8. Maintain strong attention to detail and timely follow-through
  9. Participate in mandatory meetings, trainings, and continuing education requirements
  10. Maintain all applicable nursing licenses and CEU requirements

Work From Home Requirements
This is a remote/work-from-home position.
Candidates must have:
  1. A quiet, professional workspace free from excessive background noise or interruptions
  2. Reliable high-speed internet capable of supporting required systems and communication platforms
  3. Ability to safely and securely handle HIPAA-protected information in a home office environment
  4. Ability to communicate clearly via phone and telehealth platforms
  5. Ability to utilize multiple software platforms and EHR systems simultaneously

Travel or temporary work locations must be approved in advance to ensure adequate internet connectivity and security standards are maintained.
Schedule Expectations
  1. Evening and overnight weekday coverage
  2. Every other weekend rotation required
  3. Every other holiday rotation required
  4. Additional weekend and holiday shift opportunities available based on operational need and call volume
  5. Flexible PRN scheduling opportunities available

Physical & Professional Expectations
  1. Ability to remain seated and actively engaged for extended periods
  2. Ability to effectively communicate through telephonic and virtual platforms
  3. Ability to accurately input documentation into EHR systems
  4. Must not consume alcohol or illegal substances while on duty or within 12 hours prior to scheduled shift
  5. Must maintain professionalism, responsiveness, and clinical accuracy throughout all interactions
