  • Schedule: Every Weekend
  • Days: Friday, Saturday & Sunday
  • Hours: 8:00 AM - 8:00 PM
  • Position Type: Remote / Virtual Care
  • Pay Range: $20.00 - $22.25 per hour
What You'll Do
As a Weekend Virtual Care Nurse (LPN), you will play a key role in reducing hospitalizations and improving patient outcomes through proactive remote support and clinical intervention.
Responsibilities Include:
  • Conduct virtual patient visits and clinical assessments
  • Perform triage and identify early warning signs or risk trends
  • Support chronic disease management and patient education
  • Monitor telehealth data and escalate concerns appropriately
  • Help prevent avoidable ER visits and rehospitalizations
  • Collaborate with interdisciplinary care teams
  • Document patient interactions accurately and timely
  • Participate in care planning and performance improvement initiatives
  • Support development of virtual care best practices
Qualifications
Required:
  • This is a 100% work-from-home position. Candidates MUST reside within the state of Florida due to operational, licensing, and/or business requirements.
  • Active, unencumbered Florida LPN license
  • Minimum of 2 years of home health experience
  • Experience with triage and hospitalization prevention
  • Strong clinical judgment and communication skills
  • Comfortable using telehealth platforms and EMR systems
  • Reliable internet connection and private remote workspace
Preferred:
  • Experience working with high-risk or medically complex patients
  • Knowledge of chronic disease management
  • Microsoft Office proficiency
