Job description

The Adult Congenital Heart Disease (ACHD) Program Manager is responsible for the operational oversight, programmatic growth, and coordination of programs provided by the Nemours Children's Health Adult Disease Program under the Division of Cardiovascular services. This includes other potential future programs developed within the scope of Adult Congenital Heart Disease.

Travel to Nemours TJU site required.

Primary functions include:

  1. Working closely with Program teams and organization leadership to develop, promote, execute, and maintain strategic and business plans in a direction that fosters the mission, goals, and objectives of the Program and ensures alignment with the strategic direction of the organization.
  2. Gathering and analyzing data to support Program planning and decision making.
  3. Developing and managing related budgets, and Program metrics.
  4. Communicating progress and development of the Programs' initiatives to internal and external leaders.
  5. Overseeing and providing care planning coordination, and implementation.
  6. Functioning as an advocate for Program partners including associates, physicians, leaders, families, and patients.
  7. Working with appropriate parties to understand and support legal and regulatory requirements related to programs and practice.
  8. Oversee patient scheduling coordination and implementation to ensure efficiency and effectiveness.

Essential Functions: 

  1. Review and revise strategies and business plans to best ensure targeted objectives are met.
  2. Develop and manage annual operating and capital budgets for related programs (as appropriate).
  3. Develop pro formas as needed, e.g., SIM, new clinic location, new partnership, new FTE's.
  4. Develop and maintain Program Dashboard to include: clinic utilization, patient satisfaction scores, referral capture, etc.
  5. Quarterly revenue/expense analysis (financial statements) by location.
  6. Quarterly report on  ptsACHD patients and procedures (e.g., EP, Cath, surgery) by clinical location. Report for each ACHD MRN to capture revenue, net revenue, inclusive of all downstream.
  7. Collaborate with marketing team to develop robust marketing strategies to enhance awareness and build opportunities for Programs. Participate in outreach initiatives as appropriate.
  8. Provide support: serving as an advocate for multidisciplinary teams, partnering associates, physicians, patients, and the organization. 
  9. Advocate for patient and family needs, ensuring their voices are heard in the planning and delivery of care.
  10. Represent Programs and actively participate in Program committee meetings.
  11. Ensure care provided meets and exceeds the standards for acceptable medical care and works to improve the provision of services throughout the organization.
  12. Commit to a culture of Quality Improvement (QI) and constantly employ the use of QI tools to improve processes and measure results.
  13. Serve as liaison to patients, families, referral sources for Program related communications.
  14. Build internal and external relationships, to develop and identify opportunities to leverage Nemours resources in support of the care of Program patients.
  15. Provide care coordination for cases. Ensure systems in place are adequate. Identify and address barriers and enhancements when needed. 
  • Gather necessary data (demographics, medical records, etc.).
  • Share patient medical information with appropriate clinicians for care plan development.
  • Assist in ensuring timely scheduling of diagnostic imaging and or interventions.
  • Always maintain patient confidentiality and appropriate behavior.

   16.Participation in weekly meetings to assist in monitoring communications, expectations and outcomes. 

Requirements: 

Bachelor's degree is required, preferably in a healthcare field; Master's degree in Business or Healthcare Administration is preferred 

Clinical license preferred, such as Allied Health Professional, RN, or APP.  If candidate is clinically licensed, must maintain the appropriate state licensure and obtain BLS certification from AHA within 90 days of hire.

Minimum 5 years of related experience working in an ACHD or other multi-site medical specialty (disease specific) program is required
