We are requesting a start date of August 3rd, 2026 to allow for orientation/training with current NP Schedule: Monday - Friday, 8 AM to 5 PM. 1 Hour break. 4:30 see last patient Highest volume of patients will be M-W-Th (~16 patients/day) and new patient intakes on Tu & Fr (~2 to 4). Bilingual English/Spanish proficiency is highly preferred. EMR: Scottcare Versacare Software DEPARTMENT: Cardiac Rehabilitation REPORTS TO: Director, Acute Care and Outpatient Rehabilitation, Respiratory Therapy, Vascular Access, and Wound Care Services RESPONSIBILITY FOR SUPERVISION: Technicians JOB SUMMARY: The Cardiac Rehab Coordinator will provide direct supervision for patients receiving cardiac or pulmonary rehabilitation and immediately respond to any adverse event during the provision of cardiac and pulmonary rehabilitation. The Cardiac Rehab Coordinator may participate in hospital and nursing committees as directed by the Chief Nursing Officer. The Cardiac Rehab Coordinator will work closely with the Medical Directors to evaluate need, develop plan of care and implement care for patients receiving cardiac or pulmonary rehabilitation. The Cardiac Rehab Coordinator will coordinate patient care in collaboration with the multi-disciplinary team to include exercise physiologists, rehabilitation technicians and/or licensed Respiratory Therapists. The Cardiac Rehab Coordinator will coordinate all aspects of the multidisciplinary education and training required for cardiac and rehabilitation and will ensure the psycho-social and educational needs are met. The Cardiac Rehab Coordinator will set up, monitor, and document cardiac and pulmonary rehabilitation services ensuring all regulatory components are met. The Cardiac Rehab Coordinator performs medical assessments including history and physical assessments and consults with physician regarding findings/changes in patient condition. The Cardiac Rehab Coordinator ensures compliance with all CMS requirements concerning the provision of cardiac and pulmonary rehabilitation. The Cardiac Rehab Coordinator interacts closely with the medical director of the program as well as referring physicians. The Cardiac Rehab Coordinator may diagnose medical and nursing conditions, plan and implement interventions, and evaluate patient response to therapy under the direction of and in collaboration with the supervising physician or the attending physician. With the guidance of the medical director, the Cardiac Rehab Coordinator will develop individualized exercise and treatment plans for review and approval by the medical director. .
