Job description

Physical Therapist - Home Health - Full Time - Lincoln/Lancaster County, NE

Eventide's Home Health Care in Lincoln/Lancaster County, NE is seeking a Physical Therapist to work full time 8a- 4:30p. The Physical Therapist is responsible for providing Physical Therapy services to Eventide's Home Health Care Seniors/residents/clients with physical or cognitive disabilities to maximize functional independence.

Essential Functions

  • Evaluates clients utilizing appropriate diagnostic muscle, nerve, joint and functional ability tests.
  • Performs Physical Therapy treatment procedures or delegates to a licensed Physical Therapist Assistant based on initial and on-going client evaluation.
  • Develops and implements the client's plan of care in collaboration with the physician, other consulting health care professionals, family/caregiver according to agency procedures and state and federal regulations.
  • Documents the care provided, instructions given, client's progress toward goals, and changes in condition adhering to Eventide's policies and procedures and state/federal regulations.
  • Reports significant changes in the client's condition to the physician and nursing staff.
  • Communicates and coordinates care and client's progress with other team members involved in the care.
  • Designs, implements, and evaluates therapeutic exercise programs, and instructs the client's family/caregiver/Home Health Aide in carrying out the program.
  • Consults with the attending physician and obtains a written or telephone order for Physical Therapy plan of care and any changes in the client's plan of care as needed.
  • Ensures that assigned client's clinical records are complete, timely, and reflect coordination of care.
  • Oversees and coordinates Physical Therapist Assistant and Rehabilitation Aide client care activities.

Ideal Candidate

  • AGE REQUIREMENT: At least age 18.
  • EDUCATION: Bachelor's Degree in Physical Therapy required.
  • EXPERIENCE: One year experience as a Physical Therapist, preferred. Geriatric experience, preferred.
  • CREDENTIALS: Physical Therapist license in good standing as recorded on the Nebraska State Registry required. CPR Required. Valid Nebraska Driver's license (unless otherwise allowed by law) and a good driving record required. Must possess auto insurance at or above Nebraska statutory requirements. All licenses, certifications, and permits must be kept up to date at the expense of the employee. Must follow Eventide's vaccination requirements.
