Overview

Salary Range $17.88 - $21.84 Hourly

Job Summary

As a patient navigator you will build working relationships, solve problems and support patients while they learn to self-navigate the health care system. This will include assisting in sending and following up on referrals, documentation support, and coordinating with community support services, other clinics and our internal departments. This is a patient facing position and will require time in the clinic.

Responsibilities and Essential Functions:
  • Promotes mission, vision, and values of MarillacHealth, and abides by service behavior standards.
  • Works with providers and patients in planning of patient care needs. Responsible for knowing the scheduling guidelines and being able to accommodate urgency of the appointments with specialists. Responsible for knowing the appropriate providers to schedule patients with for referrals. Performs patient education regarding their referrals and coordination of care. Responsible for referral follow-through once the appointment has been completed.
  • Works closely with clinical staff to maintain well documented and supportive patient flow with regards to referral opportunities and tests as necessary. Documents the patient information appropriately in computer systems for positive patient outcomes. Maintains and updates systems to track patient information appropriately.
  • Coordinates and works closely with other community clinical areas to support the patient for necessary tests and procedures. Familiar with community resources and maintains a positive relationship with community entities that offer support.
  • Pre-authorizations for imaging and procedures as needed. Requires great attention to detail and patient care necessities.
  • Works with patient in support of the Patient Representative department for enrollments and other required documents as needed.
  • Participates in strategic clinic meetings as necessary (i.e.: Quality improvement, EMR Committee, Clinic Committee)
  • Visits with patients as needed during the clinical hours to ensure timely prior authorization steps and to facilitate timely scheduling.
  • Maintains cohesive relationships with clinical departments, Medical Providers, and other Medical Office Staff to ensure seamless workflow within the departments (i.e. huddles).
  • Assists patients with navigation to community services such as; DHS, community outreach for financial services, legal services, migrant farmworkers, child and migrant services and specialty offices
  • Manages patient recall lists. This may include calling patients and rescheduling them for appointments to ensure proper tests or imaging have been performed. This includes handling health maintenance reports for registry purposes and relaying health maintenance results to the patients.
  • Documents the patient information appropriately in computer systems for positive patient outcomes. Maintains and updates systems to track patient information appropriately. Reports on statistical information and outcomes as required.
  • Works to ensure the timely closing of referrals.
Education / Experience:

Education / Experience Required: • One year of medical office experience, including knowledge of medical terminology

Preferred • Patient Navigator Experience: 2 years • Patient Navigator Certification • Bilingual preferred
