What You'll Do


As a Home Care Registered Nurse (RN), you'll provide skilled nursing care in patients' homes, including:


  • Conducting comprehensive nursing assessments and developing individualized plans of care
  • Completing OASIS Start of Care, Resumption of Care, Recertification, and Discharge assessments
  • Wound care and dressing changes
  • Medication management and patient education
  • Catheter, ostomy, and other skilled nursing care
  • Disease management and chronic care education
  • Coordinating care with physicians and interdisciplinary healthcare teams
  • Monitoring patient progress and identifying changes in condition
  • Ordering medical supplies as needed
  • Completing accurate, timely electronic documentation
