RN Case Manager Responsibilities

The RN Case Manager's duties include, but are not limited to:

  • Complete an initial assessment of patients and their families to determine hospice care needs.

  • Regularly re-evaluate patients' nursing needs.

  • Use assessment data to develop and update individualized care plans.

  • Collaborate with the interdisciplinary healthcare team, patients, and families to ensure coordinated, continuous care.

  • Counsel patients and families regarding end-of-life care and support.

  • Initiate appropriate preventive and palliative nursing interventions.

  • Communicate with physicians regarding patient needs and report changes in condition.

  • Supervise Hospice Aides.
