Job description

Grace Health is seeking a compassionate Integrated Behavioral Health Consultant to make a meaningful impact in our community.
We're looking for someone who is an attentive listener with a solid understanding of how to treat and manage mental, emotional, and behavioral health conditions. The ideal candidate is not only knowledgeable but also approachable and respectful in their interactions with patients.

Sign-On Bonus: $5,000

$2,500 awarded after successful completion of the probationary period, and an additional $2,500 will be awarded after 6 months.

BASIC JOB PURPOSE:
Provides evidence-informed, brief behavioral health interventions and consultation services to patients within the Grace Health primary care setting using the Primary Care Behavioral Health (PCBH) model. Works as an integrated member of the healthcare team to improve the overall physical and mental well-being of patients. Services are brief, time-limited, and focused on functional improvement and population health. May work with individuals across the lifespan, including children, adolescents, adults, and families.

SUPERVISION RECEIVED:
Reports to the Behavioral Health Manager. Clinical supervision is provided by the Behavioral Health Director and/or appropriate licensed behavioral health supervisor.

TYPICAL PHYSICAL DEMANDS:
Requires sitting for moderate periods of time. Occasional bending, stretching, or lifting. May require close proximity contact with individuals with communicable diseases. Requires the ability to work under high stress conditions for long periods. Ability to use equipment in an office or clinical environment.

TYPICAL WORKING CONDITIONS:
Fast-paced medical clinic environment with routine exposure to communicable diseases. Frequent collaboration with medical providers, nurses, and support staff in exam rooms and other shared clinical spaces.

EXAMPLES OF DUTIES: (This list may not be all-inclusive.)
Deliver brief, solution-focused interventions (typically 1520 minutes) to address a wide range of behavioral health concerns such as depression, anxiety, sleep problems, chronic pain, adherence to medical treatments, and health behavior change (e.g., smoking cessation, diet, physical activity).
Conduct same-day warm hand-offs and scheduled visits with patients referred by primary care providers.
Perform brief functional assessments using biopsychosocial frameworks and evidence-based screening tools.
Provide real-time consultation and feedback to primary care providers regarding behavioral health concerns, treatment planning, and diagnosis when appropriate.
Collaborate with medical staff to develop patient-centered care plans that support whole-person health.
Assist in care coordination and follow-up planning as part of the care team.
Support patients in developing self-management skills for chronic diseases.
Educate patients, families, and team members on behavioral health conditions and available community resources.
Participate in quality improvement initiatives and team-based care strategies.
Provide consultation to the care team on behavioral health workflows and the integration of services.
Document all encounters efficiently in the electronic health record in accordance with organizational standards.
Participate in ongoing professional development and interdisciplinary team meetings.
May engage in training and supervision of interns or limited license professionals.

PERFORMANCE REQUIREMENTS
Knowledge, Skills and Abilities:
Commitment to working in a fast-paced, team-oriented primary care setting
Excellent skills in brief, evidence-based interventions (e.g., CBT, ACT, MI, PST)
Familiarity with medical conditions, common medications, and how they intersect with behavioral health
Ability to work efficiently and flexibly with diverse populations and presenting issues
Excellent documentation and verbal communication skills
Familiarity with behavioral health screening tools (PHQ-9, GAD-7, etc.)
Knowledge of relevant Michigan reporting laws and ethical guidelines
Thorough understanding of community resources and integrated care best practices

Education:
Completed Master's Degree (at minimum) in Social Work, Psychology, Counseling Psychology, or Counseling, or Ph.D./Psy.D. in Clinical or Counseling Psychology.

Experience:
Minimum 2 years of combined pre- and post-degree experience in a behavioral health, medical, or integrated primary care setting preferred. Experience working in a PCBH or IBH model strongly preferred.

Certificate/License:
Ph.D., Psy.D., or Master's degree in a behavioral health field from an accredited program. Licensed in the State of Michigan as an LMSW, LPC, LP, or equivalent. Limited license holders must obtain full licensure within two years of employment.
