We are seeking Nurse Practitioners/Physician Assistants with Full Practice Authority to perform in-home/telehealth health assessments for patients in the Chicago area. This is a unique opportunity to work with patients who may be less engaged with traditional healthcare, helping them better understand and access the care they need.

What You’ll Do:

  • Conduct comprehensive health assessments in patient homes.
  • Review and update medical histories and current medications.
  • Document findings and care plans in EHR.
  • Identify health risks, chronic conditions, and gaps in care.
  • Travel within Chicago as needed (mileage reimbursed).
