Clinic Registered Nurse (RN)
Department: Participant Care
Employment Type: Full Time
Location: Jackson, MI
Reporting To: Natasha Whitworth
Compensation: $37.00 - $39.00 / hour
Description
Schedule: Full Time 40 hours/week | 8am-4:30pm | Monday-Friday | No weekends! No holidays!
Department: Thome PACE | Jackson, MI
What You'll Do:As a Clinic RN at Thome PACE, you'll provide direct and indirect nursing care that supports participants' health, independence, and overall well-being. In this role, you'll use your clinical judgment to assess participant needs, complete required nursing assessments, identify changes in condition, and coordinate follow-up care in collaboration with the care team.
Key Responsibilities
  • Conduct Nursing Assessments - Complete participant assessments, fall assessments, and required documentation in accordance with PACE standards.
  • Identify & Respond to Care Needs - Recognize participant concerns, changes in condition, and needs requiring follow-up or referral.
  • Develop & Update Care Plans - Support initial, semi-annual, and annual care planning using participant, family, and care team input.
  • Provide Direct Nursing Care - Administer treatments, procedures, medications, IV therapy, tube feedings, wound care, and other nursing interventions as ordered.
  • Manage Medications & Clinical Supplies - Support medication reconciliation, participant education, pharmacy communication, medication room stocking, and required logs.
  • Coordinate Participant Care - Collaborate with the care team, providers, hospitals, nursing facilities, and families to support continuity of care.
  • Support Hospital & Facility Transitions - Track hospitalizations, assist with admission/discharge communication, and attend care conferences as assigned.
  • Complete Timely Documentation - Accurately document care provided, changes in condition, interventions, infection reporting, and follow-up actions.
  • Participate in Team Collaboration - Attend IDT meetings, clinic huddles, and other team meetings to support participant care planning.
  • Support Quality & Safety - Participate in Quality Improvement initiatives, follow safety protocols, and report concerns related to care or working conditions.
  • Provide Home Visit Support - Conduct home visits as assigned for participant assessment, intervention, and follow-up care.

Skills, Knowledge & Expertise
Required Qualifications
  • Graduate of an accredited School of Nursing
  • Current Registered Nurse license in the State of Michigan
  • Current CPR certification or willingness to obtain
  • Valid driver's license and reliable transportation
Preferred Qualifications
  • Three or more years of nursing experience in a healthcare setting
  • At least one year of experience working with frail elderly or older adult populations
  • Strong assessment, critical thinking, and care coordination skills
  • Ability to communicate effectively with participants, families, providers, and care team members
  • Comfort working in both clinic and community-based settings as needed
