POSITION SUMMARY
The Director of Health Services is responsible to oversee and coordinate the care and services of Residents, promote the effective utilization and monitoring of health care resources, and lead a multidisciplinary team to achieve desired clinical, emotional and wellness outcomes in accordance with federal, state, local, company standards and established policies and procedures and as may be directed by the Executive Director. The Director ensures Quality Delivery of Resident care through the promotion of systems utilizing Quality Improvement Practices. The Director is responsible for providing input into the budget and monitoring its effective implementation through regular reporting and monitoring of resources. The Director develops staff plans which effectively recruit, allocate, retain, develop and mentors' staff to established standards of performance while building a strong rapport with those we serve and their families. The ultimate goal of quality Resident care is to enable each Resident, where possible, to function at the highest level, to attain a stage of independence, or to at least prevent regression. An active approach to Resident care will be taken and not just passive maintenance.
ESSENTIAL RESPONSIBILITIES
  1. Support mission, vision, and goals of the organization, upholding and promoting company culture and vision
  2. Establish and monitor goals and expectations for the Assisted Living and Memory Care departments
  3. Plan, direct, and evaluate the programs, activities and schedules of the Assisted Living and Memory Care departments in compliance with all applicable rules, regulations, guidelines and company practices
  4. Conduct staff supervisory responsibilities to include selection, hiring, orientation, training, scheduling, coaching, monitoring, counseling, discipline and terminations
  5. Administer Associate Policies and Procedures. Develop, maintain and periodically update written policies and procedures and job descriptions. Orient staff and train on policies and procedures
  6. Develop departmental budgets, re-forecasts and staffing plans
  7. Assist in the administration of the Property/Risk Management Procedure Plans
  8. Make reports and recommendations to the Executive Director as necessary or required concerning the operation of the departments
  9. Develop, implement and monitor the Quality Improvement, Safety, Infection, Disaster and Environmental Control Programs in concert with company and state standards and provide appropriate plans of action to correct any identified deficiencies
  10. Participate in and oversee the admission and on-going Resident assessment and care planning process
  11. Coordinate and/or conduct Resident care plan conferences with families/sponsors. Actively communicate and work with Families, Resident and staff to resolve complaints and grievances. Updates Residents and Families when appropriate to assist in safe, respectful and appropriate transitions to other care settings, as necessary. Coordinate feedback and communication with Resident/family and staff to ensure safety and appropriate level of care
  12. Act as Resident liaison and advocate to the Executive Director
  13. Act as Resident/Administrative advocate for development of and participation in Residents' Council
    for the Assisted Living and Memory Care Setting
  14. Develop methods for coordination of the departments' services with other departments and outside agencies and health professionals to ensure the continuity of the Residents' total regimen of care. Coordinate with other staff members in scheduling events and use of facilities
  15. Oversee compliance with established company and state mandated practices for documentation. Monitor and coach staff for improved performance
  16. Oversee surveys and inspections made by government agencies
  17. Complete appropriate forms, reports, evaluations, and studies. Monitor and maintain documentation compliance in all areas to include medication system, MAR's, incidents, Resident clinical information, QA, safety and required reports
  18. Assist in Resident care as needed
  19. Attend in-service classes and staff meetings
  20. Obtain and demonstrate knowledge of the Spark Book, Spark Lifestyle and Wellness philosophy and its inherent core values. Support and participate in Spark Book Culture Initiatives (e.g. assist with feedback groups, assist in new initiatives, encourage Residents and act as champions of fun)
  21. Develop job related knowledge, skills and abilities by participating in continuing educational opportunities for personal growth and development
  22. Perform other duties as assigned

QUALIFICATIONS AND EDUCATION REQUIREMENTS
  • Bachelor's Degree
  • Assisted Living Administrator License and/or other License as required by the state
  • Registered Nurse
  • CPR
  • At least 3yrs progressively responsible work experience in the senior living industry or related field
