What You'll Do
  • Provide skilled nursing care under the direction of a Registered Nurse and physician
  • Administer medications and perform treatments as ordered
  • Monitor patient conditions and report changes to the supervising RN
  • Assist with disease management and patient education
  • Complete wound care and other skilled nursing procedures within your scope of practice
  • Collaborate with physicians and the interdisciplinary care team
  • Complete timely and accurate clinical documentation
Qualifications
  • Active Virginia Licensed Practical Nurse (LPN) License Required
  • Valid Driver's License and Reliable Transportation
  • Strong Clinical, Communication, and Documentation Skills
  • Home Health Experience Preferred but Not Required
