We are currently seeking an experienced:

HOME CARE LPN OFFICE NURSE

Full Time

Job Description:

  • Provides direct patient care, to include using a variety of skills and necessary coordination of services.
  • Completes appropriate documentation as required by policy and procedure and regulation.
  • Assists Patient Care Coordinator and/or Operations Manager in day to day branch office tasks to ensure patient care documentation is complete and submitted timely.
  • Assists Patient Care Coordinator and/or Operations Manager in management of patient care supplies.
  • Manages all commercial insurance payers and Medicare replacement plans for continued authorization regarding home care visits and documents authorization in electronic medical record per software system.
  • Coordinates with Operations Manager and Patient Care Coordinator in the management of Face to Face documentation as required by specific payer source.

Job Requirements:

  • Current LPN licensure within the state of branch location, and successful completion of nursing education from an approved school of nursing.
  • One year of clinical nursing experience, preferably in an acute care medical/surgical setting.
  • One year of Home Care experience preferred.
  • Ability to independently organize work and provide effective health care services to homebound patients.
  • Effective communication skills (oral and written).
  • CPR certified.
