Key Responsibilities & Performance Indicators (Your Job):
1. Client Satisfaction & Personal Care
  • Communication Excellence: Facilitate effective communication between clients and their families, aiming for the maximum score in quarterly satisfaction surveys.
  • Personal Hygiene & Grooming: Maintain clients' hygiene and grooming standards to meet the maximum satisfaction score in related survey segments.
  • Meal Planning & Nutrition: Implement individualized meal plans that reflect dietary goals, targeting the maximum score in food and diet feedback.

2. Housekeeping & Safety
  • Environment Maintenance: Ensure living spaces are clean, safe, and comfortable, achieving the maximum score during monthly site inspections.
  • Safety Protocols: Regularly inspect and maintain adaptive equipment to ensure all safety measures are in place and receive the maximum rating in compliance reviews.

3. Medical Appointment Coordination
  • Scheduling Efficiency: Coordinate with nursing to schedule all medical appointments on time, aiming for a 100% on-time scheduling rate each month.
  • Attendance Reliability: Ensure clients attend all scheduled medical appointments, maintaining a 100% attendance rate.
  • Documentation Accuracy: Submit and file all post-appointment records promptly, targeting a 100% documentation compliance rate.

4. Community Integration
  • Activity Participation: Plan and execute a minimum of two community outings per month, submitting photographic documentation to the assigned email for maximum score in engagement assessments.
  • Transportation Support: Provide reliable transportation, ensuring availability of personal or company vehicles to support transportation needs.

5. Documentation & Compliance
  • Shift Logs: Complete and submit all shift logs accurately and on time to meet a 100% compliance benchmark.
  • Training & Credentials: Maintain up-to-date certifications and participate in all required training sessions to meet the maximum score in HR audits.

Requirements
  • Ability to drive 15-18-passenger vans
  • HCS group home experience with 3 clients
  • House Manager experience a plus
  • Experience dealing with clients who have behaviors
  • Experience in electronic documentation
  • High school diploma or GED
  • US Work Eligibility
  • Texas Driver's License
  • Auto Insurance
