Catholic Charities of Southern Missouri is seeking a compassionate and motivated Family Support Coordinator to join our Maternal & Family Programs team serving LifeHouse and Healthy Moms, Healthy Babies in Cape Girardeau. About the Programs
LifeHouse
LifeHouse provides a safe, supportive residential environment for pregnant women experiencing homelessness or crisis situations. Residents receive housing, case management, life skills education, and support as they work toward self-sufficiency.
Healthy Moms, Healthy Babies
Healthy Moms, Healthy Babies provides community-based pregnancy and parenting education designed to improve maternal and infant health outcomes while helping families achieve long-term stability.
Requirements
What You'll Do
- Assess client strengths, needs, and barriers to success.
- Develop individualized service plans that support housing stability, healthcare access, employment, education, parenting, and self-sufficiency goals.
- Meet regularly with clients to monitor progress and provide ongoing support.
- Connect families with community resources and services.
- Advocate for clients with healthcare providers, landlords, schools, employers, and community agencies.
- Help clients navigate challenges and remove barriers to success.
- Coordinate services with community partners and service providers.
- Facilitate referrals and ensure continuity of care.
- Maintain accurate and timely documentation and data entry.
- Participate in program evaluation, quality improvement efforts, and community outreach activities.
- Mentor interns, practicum students, and volunteers as assigned.
- Perform other related duties as assigned.
What We're Looking For
Required Qualifications
- Bachelor's degree in Social Work, Human Services, Psychology, Sociology, Counseling, Education, Family Studies, Behavioral Science, or a related field.
- Valid driver's license, reliable transportation, and proof of insurance.
- Strong communication, organizational, and problem-solving skills.
- Ability to work independently while collaborating effectively with a team.
- Commitment to serving women, children, and families with compassion and respect.
New to Case Management?
No prior case management experience is required. Catholic Charities of Southern Missouri is willing to train qualified candidates who demonstrate professionalism, compassion, strong communication skills, and a desire to help women and families achieve stability and self-sufficiency.
If you have experience working with people in education, healthcare, behavioral health, ministry, childcare, customer service, community outreach, corrections, nonprofit services, direct support, or other helping professions, we encourage you to apply. We provide training, mentorship, and ongoing support to help you succeed in this role.
Preferred Qualifications
- Experience providing case management or supportive services.
- Experience working with individuals experiencing homelessness, housing instability, poverty, or crisis situations.
- Knowledge of community resources, housing programs, healthcare systems, and social service networks.
- Training or experience in trauma-informed care, motivational interviewing, and strengths-based practice.
- Experience supervising or mentoring students, interns, or volunteers.
