Description:

Preston-Taylor Community Health Centers, Inc. is composed of a team of dedicated health care professionals that have at heart the mission of the organization of providing comprehensive primary health care services to the residents of the four-county service area of West Virginia in Preston, Taylor, Tucker, and Grant Counties regardless of their ability to pay. This is accomplished through the provision of primary care services that are culturally sensitive and relevant to the needs of the community.


In supporting the delivery of activities deriving from 6 Community Health Care Medical Centers, 2 Pharmacies, 2 Dental Centers, 1 Optometry Office and a School Based Health Center, we are seeking a Licensed Clinical (LCSW) or Graduate Social Worker (LGSW) or Licensed Professional Counselor (LPC) to join the organization. This position will work from the schools located in the Taylor County, West Virginia school district and with the Medical Center of Taylor County.


Primary Responsibilities:


  • Follow guidelines per HIPAA & FERPA regarding patient's PHI, OSHA, and other organizational policies regarding patient care.
  • Serves as a member system of the school team and integrates within the school-wide culture and system.
  • Provides psycho-social assessment, psychological related services to students eligible for school based behavioral health services and consultation to school staff, community agencies and community liaison teams.
  • Assist teams in developing appropriate social-emotional goals and objectives, and progress monitoring.
  • Assists with in-service training to parents, community agencies, student groups and educational staff members, relating to the promotion of positive mental health principles related to educational success.
  • Provide community referrals and regulatory compliance documentation.

Qualifications:


  • Current WV license as one of the following: Licensed Clinical Social Worker (LCSW); Licensed Graduate Social Worker (LGSW); Licensed Independent Clinical Social Worker (LICSW) or Licensed Professional Counselor (LPC).
  • Experience working with schools and public health preferred.
  • Strong computer skills and proficiency with electronic health records.
  • Strong communication skills and knowledge of the communities of which we serve.
  • Must be well organized with a strong attention to detail and accuracy.
