Lifekind Health is looking for a full-time Behavioral Health Care Manager to serve as a critical link between behavioral health, medical services, and care coordination teams. This role supports compliance with Enhanced Care Management (ECM) program requirements while improving patient engagement, continuity of care, and clinical outcomes for moderate to high-risk populations. Help to increase show rates, ensure compliance, and optimize clinical staff efficiency by shifting non-billable tasks away from licensed providers.

Schedule: Monday – Friday, 8:00 am – 5:00 pm

Location: Works out of 2 clinics in Victorville


About the position:

  • Provide Enhanced Care Management (ECM)-aligned care coordination for high-acuity patients and patients with comorbid behavioral and medical conditions
  • Conduct proactive patient outreach, engagement, and follow-up
  • Identify and address barriers to care (e.g., transportation, psychosocial factors)
  • Monitor behavioral health status and escalate clinical concerns appropriately
  • Facilitate communication across interdisciplinary care teams
  • Ensure accurate, timely, and compliant documentation per payer requirements
  • Support continuity of care across behavioral health, medical, and ancillary services


Our Ideal Candidate will have the following qualifications:

  • Master’s degree in Social Work (MSW) or Marriage and Family Therapy (MFT) (unlicensed); or Master’s in a related mental health field with an Associate Professional Clinical Counselor (APCC), Associate Clinical Social Worker (ASW), or Associate Marriage & Family Therapist (AMFT) certification
  • Basic Life Support (BLS) or CPR certification required
  • Experience in behavioral health, care coordination, or case management strongly preferred
  • Strong interpersonal and communication skills
  • Ability to work within a multidisciplinary care model
  • Knowledge of Medi-Cal and ECM program requirements preferred
  • Ability and willingness to travel to the 2 clinics in your assigned region
