Job description

GENERAL STATEMENT OF RESPONSIBILITY: Responsible for receiving and preparing Wellness, Referral Lab Services, and Clinical specimens for laboratory analysis, routing accessioned specimens to the appropriate laboratory departments, placing orders for testing in accessioning application.
ESSENTIAL FUNCTIONS:
Level 1
• Scan all Wellness, Referral Lab Services, and Clinical packages received from FedEx and other couriers.
• Open all specimen packages received for Wellness, Referral Labs, and Clinical.
• Scan and index consent form/lab requisition into imaging system.
• Scan all contents of packages (tubes and consent form/lab requisitions/lab requisition) into accessioning application.
• Recognize problem samples and report as appropriate.
• Identify client, region and reference code from consent form/lab requisition and enter into accessioning application.
• Enter demographic information (date of birth, gender, collection date and other information as appropriate) from consent form/lab requisition into accessioning application.
• Apply sample identification (barcode) as needed to all specimen types/forms.
• Place test orders from each consent form/lab requisition into accessioning application (requires extensive working knowledge to convert individual test orders indicated on consent form/lab requisition to corresponding CRL test/panel codes).
• Centrifuge, pour off and aliquot samples as necessary.
• Identify and process all send out tests which must be forwarded to external reference laboratories. This entails the accurate completion of order forms, transferring the correct sample type and delivery to designated pick up points.
• Deliver specimens to various lab departments for in-house testing as needed.
• Keep accessioning/work area clean and well stocked, dispose of trash and hazardous waste properly.
• Maintain and protect the confidentiality of all CRL, CRL subsidiaries, legal entities and client information.
• Be able to comply with all applicable federal, state, and local safety and health regulations that would apply to this job.
Level 2
• Perform all duties for Accessioning Clerk Level 1
• Proven competency in more than one discipline
o Disciplines defined as:
• Clinical/Wellness Accessioning
• Insurance Accessioning
• Sample Management
• Laboratory Assistant Suport
o Competency and proficiency will be demonstrated as follows:
• Must be trained on all processes within the discipline
• Must have completed 6 months training
• Must continually demonstrate competency through annual assessment
• Must follow all SOPs
o Failure to demonstrate proficiency and competency will result in removal of Level 2 classification.
Level 3
• Perform all duties for Accessioning Clerk Level 2
• Proven competency in more than two disciplines
o Disciplines defined as:
• Clinical/Wellness Accessioning
• Insurance Accessioning
• Sample Management
• Laboratory Assistant Support
o Competency and proficiency will be demonstrated as follows:
• Must be trained on all processes within the discipline
• Must have completed 6 months training
• Must continually demonstrate competency through annual assessment
• Must follow all SOPs
o Failure to demonstrate proficiency and competency will result in removal of Level 3 classification.
Level 4
• Perform all duties for Accessioning Clerk Level 3
• Proven competency in two or more business units
o Business units are defined as:
• General Laboratory
• Toxicology Laboratory
o Competency and proficiency will be demonstrated as follows:
• Must be trained on all processes within the discipline
• Must have completed 6 months training
• Must continually demonstrate competency through annual assessment
• Must follow all SOPs
o Failure to demonstrate proficiency and competency will result in removal of Level 4 classification.

The following physical attributes are required for this position:
• Sitting for extended lengths of time
• Close vision requirements due to computer work

• Pushing and pulling of large carts, trash bags, and specimen bags
• Lifting, up to 30 pounds
• Ability to hear auditory cues
EQUIPMENT: Laboratory coat, gloves, protective eye wear, box knife, scan gun, PC, centrifuge, barcode machine, laser printer, copier, telephone, fax machine and imaging system and trash compactor.
OTHER: Shift extends until work is finished; Overtime required according to workload; weekend work is necessary. Specimens handled consist of body fluids & must be regarded as potentially infectious. Trash compactor operator requires minimum age of 18 years.
