Treatment Nurse - LVN

Kern River Transitional Care
Full-Time
Pay Range: $28.00-$34.00 per hour, based on experience

Kern River Transitional Care is seeking a compassionate and dependable Licensed Vocational Nurse (LVN) to join our team as a full-time Treatment Nurse. The ideal candidate will have strong clinical and documentation skills, excellent attention to detail, and a commitment to providing high-quality, resident-centered care.

Key Responsibilities
  • Perform wound assessments and provide treatments according to physician orders and established care plans
  • Monitor residents for changes in skin condition, wounds, pressure injuries, and other treatment needs
  • Accurately document treatments, wound measurements, progress, and changes in condition
  • Notify physicians and appropriate nursing leadership of significant findings
  • Ensure treatment records and physician orders are complete and current
  • Collaborate with nursing staff, physicians, therapists, and other members of the interdisciplinary team
  • Assist with developing and updating individualized care plans
  • Implement pressure-injury prevention and skin-integrity programs
  • Educate residents, families, and staff regarding wound care and prevention
  • Maintain treatment supplies and ensure equipment is clean and properly stored
  • Follow infection-control practices and all applicable facility, state, and federal requirements
  • Participate in care conferences, quality assurance activities, and facility surveys as needed
Qualifications
  • Current California LVN license in good standing
  • Current CPR/BLS certification
  • Previous wound care or treatment nurse experience preferred
  • Skilled nursing or long-term care experience preferred
  • Knowledge of wound prevention, assessment, treatment, and documentation
  • Strong communication, organizational, and clinical skills
  • Ability to work effectively as part of an interdisciplinary team
  • Compassionate, reliable, and committed to excellent resident care
