Tranquil Minds Wellness is seeking a dedicated and compassionate Licensed Master Social Worker (LMSW) to join our team as a full-time member. In this role, you will have the opportunity to make a significant impact on the lives of seniors and their families by providing essential support, guidance, and resources in a hospice care setting. Work Environment

This position offers a remote work setting with a full-time schedule from Monday to Friday, 8:00 AM to 4:30 PM. While primary duties can be performed from home, occasional evening or weekend hours may be required to accommodate clientsโ€™ needs. Our flexibility in scheduling not only supports our clients but also allows for a balanced work-life dynamic for our employees. Being on-call may also be necessary to address urgent situations that arise, ensuring that our clients receive continuous care and support.

Key Responsibilities
  • Conduct thorough psychosocial assessments to evaluate the needs of our senior clients, ensuring a comprehensive understanding of their emotional, social, and mental well-being.
  • Develop and implement customized care plans in collaboration with clients and their families, aiming to achieve their specific goals and enhance their quality of life.
  • Provide counseling and emotional support to clients and their families as they navigate the challenges associated with aging and end-of-life transitions.
  • Advocate for clientsโ€™ rights and facilitate access to essential community resources, ensuring they receive appropriate support and services tailored to their unique situations.
  • Collaborate with an interdisciplinary team of healthcare professionals, including nurses and caregivers, to coordinate care and address clientsโ€™ holistic needs effectively.
  • Conduct regular follow-up visits to monitor clientsโ€™ progress, assess their living conditions, and address any emerging concerns regarding their safety and well-being.
  • Facilitate communication and resolution of conflicts among family members involved in the care of senior clients, ensuring the maintenance of a supportive family dynamic.
  • Educate clients and their families about available community resources, support groups, and programs that can enhance their engagement and overall quality of life.
  • Maintain comprehensive and accurate documentation of client assessments, care plans, and significant interactions as per regulatory standards and organizational policies.
  • Stay informed about current best practices in social work, ethics, and policies to promote compliance and integrity in service delivery.
Patient Population / Client Focus

You will primarily work with seniors in a hospice setting, understanding their unique challenges, emotional needs, and healthcare preferences. Each client presents distinct life experiences, requiring you to be an empathetic listener and a resourceful problem-solver. By focusing on individual goals, you will help clients navigate the complexities of aging while respecting their dignity and autonomy.

Qualifications
  • A Masterโ€™s degree in Social Work from an accredited institution.
  • Current and valid state licensure as a Licensed Master Social Worker (LMSW).
  • A minimum of two years of experience as a Social Worker, preferably in a geriatric or community healthcare setting.
  • In-depth knowledge of social work principles, aging-related challenges, and available community resources.
  • Strong assessment, counseling, and problem-solving skills, with a commitment to empathetic listening and understanding.
  • Excellent interpersonal and communication abilities, facilitating effective teamwork across multidisciplinary settings.
  • Proficiency in electronic health record (EHR) systems and familiarity with essential computer applications.
Preferred Experience
  • Experience working in hospice care or with geriatric populations is highly desirable.
