Who You Are: You're a Care Coordinator looking for an opportunity to define your own career path, connect with a supportive community of health professionals like you, and increase your earning potential. You have a passion for working in different care settings and making a difference in lives.
What will you do? As a Care Coordinator at Aspire Health Partners, your job purpose will be to provide a variety of essential functions to coordinate care for program participants.
These functions include:
  • Providing resource linkage, connection and advocacy to program participants.
  • Maintaining up-to-date knowledge of area resources and how to access them.
  • Working with multi-disciplinary teams to gather information and provide the best wraparound care possible for clients.
  • Documenting clearly and effectively in the Electronic Health Record.
  • Following all program specific protocols in relation to specified role, maintaining fidelity to program model.
  • Maintaining all required credentials necessary to perform job functions.
Qualifications
  • Bachelor's Degree in Social Services
  • 1 year working with target population required for some positions
