Back on Track Counseling & Consulting is seeking a dedicated Licensed Clinical Social Worker (LCSW) to join our team in a full-time, remote capacity, focusing on providing exceptional mental health services within a Hospice Center setting. Regular communication with colleagues, patients, and families is essential, aided by a state-of-the-art electronic health record (EHR) system that facilitates documentation and interdisciplinary collaboration.

Key Responsibilities

As a Licensed Clinical Social Worker (LCSW), your day-to-day responsibilities will include:

  • Conducting comprehensive psychosocial assessments to identify patients’ needs, strengths, and challenges, including the evaluation of their emotional and mental health in the context of their chronic illnesses.
  • Providing individualized counseling and supportive therapy to patients and their families, focusing on disease management, coping strategies, and health education tailored to their specific conditions.
  • Collaborating with patients and an interdisciplinary healthcare team to develop and implement personalized care plans aimed at behavior modification, lifestyle changes, and the promotion of mental well-being.
  • Engaging in regular outreach to patients based on their preferences and needs, facilitating consistent communication and encouraging treatment adherence to enhance overall health outcomes.
  • Maintaining accurate and timely documentation of patient interactions, progress notes, and care plans within the electronic health record (EHR) system to ensure continuity of care and data accuracy.
Patient Population / Client Focus

You will work primarily with patients in hospice care, focusing on individuals facing terminal illnesses and their families. This population often requires specialized mental health support as they navigate complex emotional landscapes related to grief, loss, and chronic health challenges. Your expertise will be vital in helping these patients and their families cope with their circumstances, ultimately aiming to enhance their quality of life during sensitive times.

Qualifications

The ideal candidate will possess:

  • A Master’s degree in Social Work from an accredited program.
  • Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Pennsylvania.
  • A minimum of three years of professional experience in a mental health or outpatient setting, with a focus on bereavement, end-of-life care, or chronic illness management.
