Position Overview

We are seeking a dedicated and experienced Senior Licensed Clinical Social Worker (LCSW) to join our compassionate team at Back on Track Counseling & Consulting. This full-time role will focus on delivering high-quality mental health support to patients within a hospice setting, ensuring that each individual receives the care and compassion they deserve during challenging times.

Work Environment

This role is primarily remote, providing flexibility and convenience while allowing you to connect with patients who are facing complex emotional and psychological challenges in the comfort of a hospice center. Our state-of-the-art electronic health record (EHR) system streamlines communication and documentation, facilitating a seamless workflow that enhances patient care. As part of our commitment to staff well-being, we encourage a healthy work-life balance with a structured Monday through Friday schedule from 9:00 AM to 5:00 PM.

Key Responsibilities
  • Conduct thorough psychosocial assessments to identify the needs, strengths, and challenges of each patient, applying an evidence-based approach tailored to the hospice setting.
  • Deliver comprehensive counseling, disease management support, and health education to patients and their families, addressing both psychosocial and emotional needs.
  • Collaborate with patients and a multidisciplinary healthcare team to formulate and execute personalized care plans, emphasizing behavior modification, coping strategies, and lifestyle adaptations.
  • Engage with patients regularlyโ€”both synchronously and asynchronouslyโ€”to support their goal-setting in managing chronic health conditions and associated mental health challenges.
  • Document all patient interactions and updates promptly using our electronic health record system, ensuring accuracy and compliance with health regulations.
  • Administer outreach initiatives tailored to patient preferences, fostering strong engagement and encouraging meaningful behavior changes.
Patient Population / Client Focus

The primary patient demographic will include individuals facing end-of-life challenges, families dealing with the complexities of hospice care, and patients requiring long-term mental health support. You will have the opportunity to work with diverse cultural groups, enhancing your cultural competence while addressing the unique mental health needs of each individual.

Qualifications
  • A Master's degree in Social Work from an accredited program is required.
  • Possession of an active state license as a Licensed Clinical Social Worker (LCSW).
  • A minimum of 3 years of experience in mental health or outpatient environments, particularly in roles related to hospice care or chronic illness management.
  • Strong familiarity with evidence-based therapeutic modalities and best practices in mental health treatment.
  • Exceptional interpersonal, communication, and assessment skills that enable you to build rapport with patients and their families effectively.
Preferred Experience
  • Experience working within a hospice or palliative care setting is highly desirable.
  • Knowledge of grief counseling and support techniques.
  • Familiarity with the healthcare regulations and standards governing hospice care.
Compensation and Benefits

We offer a competitive salary range of USD 90,000 to 100,000 per year, supplemented by performance-based bonuses.
