As a Home Health Licensed Practical Nurse (LPN), here's a big-picture view of what you'll do:
  • Provide home-based care under the supervision of a RN to patients with illnesses, injuries and chronic diseases-often after discharge from a hospital, rehab or nursing facility
  • Work with a team of physicians, RN's, and CNAs to deliver quality patient care
  • Examine patients, complete assessments, document progress and report changes to their physician
  • Educate patients on their plan of care, prescribed medication, therapy, diet and exercise
  • Coach family members on the patient's plan of care, medication and home safety
  • Collaborate with interdisciplinary team to coordinate patient care and ensure goals are met and plan for discharge from services
A few must-haves for Home Health LPN's:
  • Graduate of a practical (vocational) nursing program for LPN and has a current GA LPN license
  • Minimum of 1 year (LPN) experience in home healthcare
  • HCHB charting experience highly preferred
  • CPR certification, negative ppd screening or chest x-ray
  • Drivers license, reliable transportation & auto insurance
  • Knowledge of state and federal home health regulations
  • Clinical proficiency in nursing specialty, strong communication skills and compassion
  • Able to pass federal and state required criminal / abuse background checks and drug screen
