Job Overview
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our hospice team. This role is crucial in coordinating patient care, ensuring quality comfort outcomes, and facilitating communication between patients, families, and healthcare providers. The ideal candidate will have experience in a hospice setting, and possess a strong background in hospice eligibility guidelines and symptom management.

Responsibilities

  • Coordinate patient care across the interdisciplinary team and services to ensure continuity of care.
  • Conduct thorough diagnostic evaluations and assessments to develop individualized care plans.
  • Manage medical records efficiently, ensuring accuracy and confidentiality.
  • Collaborate with multidisciplinary teams to provide comfort care for patients with complex needs, including those requiring infectious disease management.
  • Facilitate communication between patients, families, and healthcare providers to enhance understanding of treatment plans.
  • Monitor patient progress and adjust care plans as necessary based on clinical outcomes.
  • Provide education to patients and families regarding hospice management and available resources.
  • Participate in quality improvement initiatives to enhance patient safety and hospice care delivery.
  • Will cover parts of Paulding, Bartow, south Cherokee Counties, Fulton, and  mileage reimbursement is provided.                               Requirements
    • Current Registered Nurse (RN) license in the state of practice.
    • Experience in a clinical setting; familiarity with hospice is required.
    • Proficient in medical record management and documentation practices.
    • Strong skills in diagnostic evaluation and patient assessment.
    • Ability to work collaboratively within a multidisciplinary team environment.
