Your Role
As the Integrated Health RN, you will play a vital role in the Colbert Comprehensive Transition Grant, supporting clients transitioning from nursing homes to independent living within the community. This position involves providing direct care and medical support, including home visits and coordination of community-based services to ensure clients can maintain their independence. You will leverage your expertise to address clients' medical needs, promote overall wellness, and provide education on medication management, helping clients achieve a stable and healthy transition to community living.
Responsibilities
  • Facilitate off-site and on-site educational services that focus on the influence of physical/mental health, the purpose and side effects of medications, and proper medication adherence.
  • Communicate with clients and insurance companies, collect data from patients to assist in managing
    their care, and coordinate/collaborate with other medical providers for continuity of care.
  • Request and review medical documentation from previous/current providers for continuity of care.
  • Provide medical consultation with Comprehensive Assessments to determine readiness for Colbert/Williams members to transition into the community
  • Provide medical support with consumers in a natural environment including but not limited to home visits, doctor appointments, and nursing homes.
  • Conduct home visits to assure all basic needs are met. Provide education on medication management and medication adherence along with administrating medications in the community.
  • Coordinate and accompany clients on medical appointments and emergency appointments if needed.
  • Spend 60% of work time providing services off-site.
Qualifications
  • Registered Nurse (RN) required.
  • Minimum of a Bachelor's Degree in Nursing (BSN)
  • Minimum of one year of professional RN experience.
  • Valid driver's license and documentation of current auto insurance, with an acceptable motor vehicle report.
  • Effective communication skills, both written and oral.
  • Proficient in the use of computers, software applications, and working knowledge of Microsoft Office Suite programs.
