About Caya Integrated Health
Caya Integrated Health is a growing integrative healthcare organization focused on providing high-quality, patient-centered care through a collaborative and holistic approach. Our team combines primary care and behavioral health services to create comprehensive treatment plans that improve the overall well-being of our patients.
We are seeking an experienced and compassionate Family Medicine Nurse Practitioner (NP) to join our team. This is an excellent opportunity for a provider seeking flexibility, autonomy, and the ability to grow with an expanding organization, with the potential opportunity to transition into a full-time role in the future.
Position Summary
The Family Medicine NP will provide comprehensive outpatient primary care services for patients across the lifespan. The ideal candidate is confident, patient-focused, collaborative, and comfortable working within an integrated healthcare environment.
This role is ideal for a provider who values quality patient care, relationship-building, and a supportive clinical environment.
Pay: 1099 / Independent Contractor — $75.00 per hour
Schedule: Available 2–3 days per week
Future Opportunity: Full-time employment may be considered in the future based on business needs and mutual fit
Responsibilities
  • Conduct comprehensive patient assessments, evaluations, and follow-up visits
  • Diagnose and treat acute and chronic medical conditions
  • Develop individualized treatment plans
  • Order and interpret diagnostic tests and labs
  • Prescribe medications and treatments in accordance with Florida law
  • Maintain accurate and timely medical documentation within the EMR system
  • Collaborate with behavioral health providers and interdisciplinary teams
  • Educate patients on preventative care and wellness strategies
  • Ensure compliance with all clinical and regulatory standards

Qualifications
  • Active Florida APRN license
  • Board Certified Family Nurse Practitioner (FNP)
  • Active DEA license
  • Minimum of 2 years of Family Medicine or Primary Care experience preferred
  • Strong clinical judgment and communication skills
  • Ability to work independently while collaborating with a multidisciplinary team
  • Bilingual (English/Spanish) strongly preferred but not required
