Job description

Care Coordination Integrated Case Management Rn Co-Coordinator

The Care Coordination Integrated Case Management (CCICM) RN Co-Coordinator is an administrative position that works in partnership with the CCICM Social Work Co-Coordinator to effectively ensure that the Tomah VA RNs and Social Workers provide case management and care coordinator adhering to the CCICM framework. The CCICM RN Co-Coordinator is responsible for the following:

  • Administratively responsible for the clinical programming and daily operations of the CCICM framework.
  • Participates in the development of policies and procedures related to sustainment and improvement of the CCICM framework
  • Collaborates with nursing and social worker services for all planning, development, education, implementation, monitoring, and evaluation of the CCICM operations.
  • Develops and tracks the facility wide CCICM education plan and provides guidance and support in the maintenance of the CCICM framework.
  • Serves as a mentor and role models the CCICM framework to nursing staff who case manages and provides care coordination to the Veteran population.
  • Provides direct patient care services while serving as a Lead Coordinator for a small panel of patients when clinically indicated.
  • Ongoing support of the CCICM Office of Rural health Enterprise-Wide Initiative (ORH EWI) which includes at least 51% of CCICM duties to be related to the care for rural and highly rural Veterans.
  • Other duties, as assigned.
