This full-time position offers an annual salary ranging from USD 87,000 to 105,000. This role is designed to be fully remote, allowing you to engage with clients from the comfort of your own home while maintaining a flexible work schedule based on patient appointments. • Conduct patient intakes, performing diagnostic and functional evaluations to develop effective treatment plans tailored to each individual's needs.
• Offer brief, focused therapeutic interventions for patients requiring mental health services, ensuring adherence to established Agency and Federally Qualified Health Center (FQHC) guidelines.
• Collaborate with primary care providers to recognize and manage mental health and psychosocial issues, providing recommendations for necessary referrals to specialty services.
• Monitor patient progress by initiating follow-ups and adjusting treatment plans as needed to reflect any changes in the patient’s condition.
• Assist in the development of relapse prevention plans, empowering patients to maintain stable functioning and prevent worsening of their mental health challenges.
• Document patient progress meticulously in treatment charts, ensuring that all records are up-to-date and informative for ongoing treatment.
• Educate patients on self-management techniques, offering them necessary skills for active participation in their recovery journey.
• Consult with management and team members regarding the effectiveness of behavioral services and potential areas for program evaluation and improvement.
• Provide support and guidance to patients in complying with medical treatments initiated by primary care providers, including strategies for managing side effects of medications.
• Adhere strictly to the Code of Ethical Conduct for Clinical Professionals while carrying out additional duties as assigned.

Your work will primarily focus on diversely populated clients who range in age and background. You will have the opportunity to engage with individuals facing a variety of mental health challenges, including anxiety, depression, trauma, and relationship issues. By understanding the unique cultural context of each patient, you will tailor your approach to achieve the best possible therapeutic outcomes.

• Master's Degree in Clinical Counseling or related field.
• Current Licensed Clinical Professional Counselor (LCPC) license in the state of Illinois.
• Thorough understanding of mental health assessments, treatment planning, and intervention methodologies, including family therapy, group therapy, and child psychotherapy.
• Ability to evaluate the individual needs of patients effectively and implement appropriate care strategies.
• Strong interpersonal skills with the ability to build cooperative relationships with patients, families, and other professionals.

• Fluency in both English and Spanish is preferred but not required.
• Experience working in multidisciplinary healthcare teams and in community health settings.
• Familiarity with DSM-IV diagnoses and psychotherapeutic techniques will be advantageous.
