The Sustainable Housing Case Manager - HUD provides case management services to clients currently receiving ACTS Housing Services and community participants who have been provided housing services through ACTS.

Position Title and Department: Sustainable Housing Case Manager; Housing Services
Pay: $56,160.04 per year ($1000 additional per year for bilingual capability in English and Spanish, French or Arabic and $1000 additional per year for additional degree of value to ACTS.) This position is contingent on grant funding, with continuation based on performance, organizational needs, and funding availability.
Reports to: Sustainable Housing Supervisor
Hours per week and Schedule: 40 hours Monday - Friday 8:00 a.m. to 4:00 p.m. Flexibility for client needs or events require
FLSA Classification: Exempt

Essential Functions:

Assessment/Intake
  1. Conducts comprehensive initial screening process to determine eligibility for program services, providing assessment, intake and enrollment where appropriate.
  2. Assist client in developing Individual Service Plan (ISP), assess progress and problem-solving barriers to self- sufficiency regularly.
  3. Monitors client's compliance with Program Agreement.

Case Management Service Delivery
  1. Serves as an advocate and liaison for assigned caseload clients receiving ACTS Housing Services.
  2. Provides education and information in building client self-sufficiency around family systems, housing, budgeting, employment, educational training, health and other human services with an emphasis on long-term housing and economic stability.
  3. Makes referrals to other services as needed and works with other community partners to provide collaborative services to clients.
  4. Executes their assigned program funding within the approved budget and in compliance with funding source requirements.

Evaluation
  1. Enters and updates information into the Homeless Management Information System (HMIS), SharePoint and other client databases as required.
  2. Maintains client files in accordance with ACTS guidelines and other accrediting agencies for program and grant requirements.

Training/Supervision
  1. Participates in regular supervision with Program Supervisor by providing regular updates and preparing case presentations.
  2. Completes required 40 hours of training annually including CPR, First Aid and NARCAN certification and other human service trainings.

Outreach
  1. Promotes community awareness about homelessness and human services.
  2. Attends staff meetings, community meetings, training and other agency related activities as required.
  3. Conducts ongoing outreach to establish/foster a network of community resources and services.
  4. Performs other duties as required.

Required Knowledge, Skills and Abilities:

Preferred Knowledge, Skills and Abilities:
  • Excellent customer service skills.
  • Preferred - CPR and First AID Certification
  • Proficient with Microsoft Office Suite
  • Bilingual capability (English and Spanish, French or Arabic)

Education and Experience:
Meets one of the following three criteria, required:
  • A bachelor's degree in human services, behavioral sciences or allied field (including criminal justice, education and psychology)
  • AA degree in human services, behavior sciences or allied field (including criminal justice, education and psychology) with 4 years of documented related experience
  • No college degree in human services, behavioral sciences or allied field (including criminal justice, education and psychology) with 5 years of documented related experience.
  • A bachelor's in human services or behavioral sciences, preferred

Physical Requirements:
  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift up to 15 pounds at times.
