Job description

ABOUT THIS OPPORTUNITY

Schedule/Hours:    Monday – Friday, 8:30AM-5PM

This position is responsible for:

Under the general supervision of the Corps Officer or the Social Service Director, the Case Manager is responsible for implementing a strengths based approach to his/her client caseload. He/she must approach their caseload with a spirit that people can and do recover from the life traumas that have brought them to the program and that his/her commitment to a strengths based approach to case management can significantly improve the client’s chances of  success. The incumbent will conduct assessments and intake; establish a healthy, professional relationship and get to know the client to assist him/her in developing and implementing personal, familial, educational, career, housing, financial and spiritual goals. He/she will identify personal and environmental strengths and consciously assist the client in exercising his/her own power, establishing their identity, making their own decisions and exercising their right to fail while charting the progress towards successful goal attainment. The incumbent serves as advocate for the client in order to acquire services and identify and participate in activities that will enable them to reach their outcomes. He/she will maintain client files and records in a manner consistent with a strengths base approach and social work best practices.

Key Responsibilities:

Essential functions – no more than 4-5 (bulleted list format).

  • Conduct client meeting; establish a collaborative relationship that emphasizes client engagement as the primary contribution for successful outcomes. 
  • Provide advocacy services by directing clients to specific service providers; increase and explain choices and encourage clients to make their own decisions and informed choices. 
  • Maintain accurate and up to date client files and case management notes including completion of client narratives, case plans, incident reports, client logs, and any and all other paperwork relevant to delivery of service. Maintain client case files in a professional and confidential manner
  • Initiate contact with community agencies (Social Services, Health Department, school system, etc.) on behalf of clients.  Collaborate with services providers; coordinate delivery services through case reviews and team meetings to ensure goals are implemented based on the case plan.

Physical Requirements and Working Conditions:

Work is performed in a normal office environment where there are little or no physical discomforts associated with changes in weather or discomforts associated with noise, dust, dirt, and the like.

WHAT WE ARE LOOKING FOR IN YOU

  • Associates' Degree in Social Work, Human Services or related field AND
  • 1-2 years’ experience performing Social Work utilizing a strengths base approach to case management or experience providing direct case management social services including accessing clients’ needs and developing individual, comprehensive, long-term action plans for recovery utilizing a wide variety of resources, OR
  • Any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.
