Location: Valparaiso, Indiana
Job Type: Full-Time with supervisor on-call rotation every 3 weeks

VNA Hospice NWI is seeking an experienced and compassionate Patient Care Manager (RN) to lead clinical operations at our 6-bed inpatient hospice care center. This leadership role is responsible for overseeing the delivery of exceptional patient care, supporting clinical staff, ensuring regulatory compliance, and fostering a culture of excellence in hospice services.


The ideal candidate is a strong clinical leader who is passionate about end-of-life care and dedicated to supporting both patients and caregivers in a collaborative, mission-driven environment.


Position Summary

The Patient Care Manager is responsible for the day-to-day clinical management of the inpatient hospice center. This position provides leadership, supervision, and support to nursing staff and other direct care team members while ensuring the highest standards of patient-centered hospice care.


Key Responsibilities
  • Provide leadership and oversight for clinical operations within the inpatient hospice center.
  • Supervise, mentor, and support nursing staff, CNAs, HHAs, and other direct care team members.
  • Ensure quality patient care through ongoing assessment, care planning, and performance monitoring.
  • Collaborate with physicians and the interdisciplinary team to coordinate patient care and symptom management.
  • Monitor compliance with hospice regulations, accreditation standards, and agency policies.
  • Assist with staffing, scheduling, and resource management.
  • Lead quality improvement initiatives and promote best practices in hospice care.
  • Participate in staff recruitment, onboarding, training, and performance evaluations.
  • Address patient, family, and staff concern promptly and effectively.
  • Maintain accurate clinical documentation and support regulatory readiness.
Requirements:


Qualifications
  • Current Indiana Registered Nurse (RN) license required.
  • Bachelor of Science in Nursing (BSN) preferred.
  • Minimum of three years of nursing experience required.
  • Previous hospice, palliative care, home health, or end-of-life care experience strongly preferred.
  • Prior supervisory or management experience preferred.
  • Strong leadership, communication, and problem-solving skills.
  • Knowledge of hospice regulations, Medicare Conditions of Participation, and quality standards.
  • Ability to lead with empathy while maintaining clinical excellence.
