Job description

Arms Acres is looking for a Clinical Case Manager to join our inpatient team in Carmel, NY. In this role, you will provide comprehensive clinical case management services that include: assessments, individual, group and family treatment, treatment plan coordination, referent relations, discharge planning, and intervention with client support systems, including social service programs and managed care.

DUTIES AND RESPONSIBILITIES:

  • Provides/ensures comprehensive evaluation of patient's functioning as defined by facility policy and procedures
  • Coordinates the development and implementation of the multidisciplinary treatment plan; coordinates treatment plan review; performs these activities within established time frames
  • Documents patient progress and own interventions in case record and works within time frames of regulatory requirements
  • Provides individual, group and family treatment (psycho-education, life skills) as identified in patient treatment plans and requested by program director
  • Performs utilization review, provides progress reports and develop aftercare plans in conjunction with referring persons/agencies, EAPs and managed care/insurance entities
  • Participates in facility program and quality improvement activities
  • Other duties as assigned

REQUIRED QUALIFICATIONS:

  • Casac -T, Casac, LMSW, LMHC
  • Master's Degree in related field
  • Experience working specifically with outpatient substance abuse counseling preferred
