Job Description:
  • Responsible for addressing the psychosocial needs of patients by utilizing consultative services, psychosocial assessment, and comprehensive intervention plans.
  • Coordinates the treatment planning process for patients in the pre-, intra-, and post- care settings, as feasible.
  • Coordinates follow up services post discharge.
  • Collaborates with other members of the treatment team to assure the continuum of care.
  • Collaborates with other community partners and agencies to ensure a safe discharge plan for patients.

Qualifications:
  • Bachelor's degree in Social Work required. Master's degree preferred.
  • Wisconsin Social Work license required.
  • Skills in assessing, planning, and managing patient care in an acute care hospital or other health care facility recommended; background in care management, and discharge planning is beneficial; knowledge of available community resources to assure the continuum of care is beneficial.
