Position Summary
The Physical Therapist assesses, plans, organizes, and participates in rehabilitative programs that improve mobility, relieve pain, increase strength, and restore functional independence for patients in the home setting.
Responsibilities
As a Physical Therapist, you will:
  • Perform and document comprehensive patient evaluations
  • Develop individualized treatment plans tailored to patient goals
  • Implement therapy programs to restore function and improve mobility
  • Monitor patient progress and modify care plans as needed
  • Educate patients and caregivers regarding treatment and safety
  • Collaborate with physicians and interdisciplinary care teams
  • Maintain accurate and timely clinical documentation
  • Ensure continuity and quality of patient care
  • Follow infection control and universal precaution guidelines
Qualifications
  • Graduate of an accredited Physical Therapy program
  • Active Florida Physical Therapist license
  • Minimum 1 year home health experience preferred
  • Current CPR certification
  • Valid driver's license and reliable transportation
  • Current auto insurance
  • HomeCare HomeBase experience preferred
Physical Requirements
  • Ability to stand, walk, bend, kneel, squat, and climb for extended periods
  • Ability to lift, carry, push, or pull up to 100 lbs.
