About the Role The Personal Service Coordinator (PSC) I/II is responsible for coordinating the needs of clients and families assigned to their caseload. The role ensures individualized care planning, facilitates access to community resources, and monitors the effectiveness of services. PSCs provide professional support through client contact, guidance, and instruction, maintaining documentation and compliance with program and contractual requirements.
