Social Services Assistant

Location: Community Extended Care Hospital of Montclair, Montclair CA

Employment Type: Full-Time

Position Summary

Community Extended Care Hospital of Montclair is seeking a compassionate, organized, and motivated Social Services Assistant to support our Social Services Department in providing quality psychosocial services to residents in our skilled nursing facility. Under the direction of the Social Services Director, the Social Services Assistant helps residents and families navigate the admission process, adjustment to long-term care, discharge planning, and access to community resources while promoting residents' rights, dignity, and quality of life.

Essential Duties and Responsibilities
  • Assist the Social Services Director with resident assessments and psychosocial documentation.

  • Help coordinate admissions, transfers, and discharge planning.

  • Meet with residents and families to provide emotional support and resource information.

  • Assist with completion of required social service documentation in accordance with state and federal regulations.

  • Coordinate appointments and referrals to community agencies and outside services.

  • Participate and manage interdisciplinary care plan meetings with residents and families.

  • Advocate for resident rights and assist with grievance resolution.

  • Help coordinate resident and family meetings as needed.

  • Maintain confidential resident records in compliance with HIPAA.

  • Support residents with adjustment to the facility and identify psychosocial needs.

  • Assist with obtaining identification documents, insurance information, and community benefits when appropriate.

  • Participate in quality improvement initiatives and regulatory surveys.

  • Perform other duties as assigned by the Social Services Director or Administrator.

QualificationsRequired
  • High school diploma or equivalent.

  • Minimum one (1) year of experience in healthcare, social services, customer service, or a related field.

  • Strong communication and interpersonal skills.

  • Excellent organizational and time management abilities.

  • Basic computer proficiency, including Microsoft Office and electronic medical records.

  • Ability to maintain confidentiality and exercise sound judgment.

Preferred
  • Associate's or Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or a related field.

  • Experience working in a Skilled Nursing Facility (SNF), rehabilitation center, or long-term care setting.

  • Bilingual (English/Korean/Spanish) is a plus.
