Health / Human Services Case Manager I
State of Delaware – Department of Health & Social Services (DHSS)
Smyrna, Delaware • Monday–Friday, 8:00am–4:30pm • $20.00/hour
Are you passionate about helping people access the care and support they need? Join a mission-driven team making a real difference in the lives of Delawareans. In this role, you’ll work directly with clients to connect them with Long Term Supports and Services (LTSS) Medicaid—helping individuals age in place with dignity and independence.
What You’ll Do
  • Serve as a trusted advocate—guide clients through the LTSS Medicaid application process from start to approved decision, gathering and managing required documentation along the way.
  • Conduct in-home visits to assess clients’ needs and deliver personalized, person-centered support.
  • Manage an active caseload—determine initial and ongoing eligibility for services and maintain accurate, up-to-date records.
  • Make referrals and connect clients with community resources to support their health, social, and economic well-being.
  • Respond to calls and emails within 2 business days and meet all activity due dates.
  • Navigate state systems daily including TAP (Tracking Assessing Planning), AWW (ASSIST Worker Web), DMES (Delaware Medicaid Enterprise System), Vital Statistics, and Outlook Calendar.
  • Check the New Referral saved search daily and initiate phone or in-person applications with new participants.
What We’re Looking For
  • Minimum 6 months of case management experience in health, human services, or a related field.
  • 6+ months of experience contributing to client service plans (clinical treatment, counseling, or benefits/eligibility determinations).
  • 6+ months of experience using an automated information system to enter, retrieve, and manage data.
  • Strong narrative writing skills—able to document application processes, write objective case notes, and complete logs and reports.
  • Skilled interviewer—able to assess needs, analyze situations, and build rapport with diverse clients.
  • Knowledge of Federal/State eligibility requirements and community resources.
  • Valid driver’s license and ability to operate a State vehicle.
  • Must pass a pre-employment drug screening.
