Job description

Position Summary:

The Social Services Assistant supports the Social Services Director in planning, organizing, and implementing social service programs to meet the emotional, social, and psychological needs of residents in a 100-bed Skilled Nursing Facility within a CCRC setting. This role ensures compliance with federal, state, and local regulations while promoting resident rights and quality of life.


Key Responsibilities:
  • Resident Support & Advocacy

    • Assist residents and families with psychosocial needs, care planning, and discharge planning.
    • Provide emotional support and help residents adjust to facility life.
    • Advocate for resident rights and ensure dignity and respect in all interactions.
  • Care Planning & Documentation

    • Participate in interdisciplinary care plan meetings and contribute to assessments.
    • Maintain accurate and timely documentation in compliance with regulatory standards.
    • Assist with MDS (Minimum Data Set) sections related to social services as directed.
  • Family & Community Relations

    • Communicate effectively with families regarding resident care and facility resources.
    • Coordinate family meetings and support groups as needed.
    • Assist in arranging community resources and services for residents.
  • Regulatory Compliance & Quality Assurance

    • Support the Social Services Director in meeting state and federal regulations.
    • Participate in audits and quality improvement initiatives.
    • Maintain confidentiality and adhere to HIPAA guidelines.
  • Team Collaboration

    • Work closely with nursing, therapy, and activities departments to ensure holistic care.
    • Assist in resolving resident concerns and grievances promptly.

Qualifications:
  • Associateโ€™s or Bachelorโ€™s degree in Social Work, Human Services, or related field preferred; or SSD with the State of Kansas
  • Experience in long-term care or healthcare setting strongly preferred.
  • Knowledge of state and federal regulations for SNFs and CCRC environments.
  • Strong communication, organizational, and interpersonal skills.
  • Ability to work collaboratively in a team-oriented environment.

Physical & Working Conditions:
  • Ability to sit, stand, and walk for extended periods.
  • Occasional lifting up to 25 lbs.
  • Work in a healthcare environment with exposure to residents and families.
